I have read the application form in its entirety. I, *applicant*, hereby indemnify and hold Fatherheart Ministries, and it’s officers, staff, representatives, and employees in conjunction with the Fatherheart Ministries “A” School harmless for any damages, injuries, and loss or damage to personal property incurred from my participation in any activities associated with this event now or during the terms of agreement. I understand that in case of illness, injury, accident, or any other damage to my person or property while participating in the “A” School which may require attention by a physician and/or hospitalization; I will bear the expense personally or by insurance that I have provided for myself. Any other cost or damages such as the cost of transportation by an emergency vehicle or damages to third persons, is also to be paid by me or by my own insurance. I release my image to be used for Social Media posts as well as future advertising for schools as per Living Loved’s discretion. I give permission to be included in occasional email mailings. (Par la présente, je, « candidat », indemnise et dégage de toute responsabilité Fatherheart Ministries, ainsi que ses offres, son personnel, ses représentants et ses employés, conjointement avec le cours ‘A’ de Fatherheart Ministries, en cas de dommages, de blessures, de pertes ou de dommages à des biens personnels résultant de ma participation à des activités associées à cet événement ou pendant la durée de l’accord. Tout autre coût ou dommage, tel que le coût du transport par un véhicule d’urgence ou les dommages causés à des tiers, est également à ma charge ou à celle de mon assurance. J’autorise l’utilisation de mon image dans les médias sociaux ainsi que dans toute publicité future d’événements, à la discrétion de Living Loved. Je donne la permission d’être inclus dans leur liste d’envois occasionnels. Je reconnais que je comprends le contenu de cette déclaration.
